This document functions as a contract between you, the Client, and Alexa Zuber and all affiliated companies in reference to The Rooted RD. When attending events, purchasing services or soliciting advice from The Rooted RD, you acknowledge that you have fully read, understood and agreed to these terms and conditions. The Rooted RD reserves the right to change these terms at any time. Your usage of the website and contact forms, and any purchases made confirms your acceptance of these policies. Please seek clarification on any content that is not fully understood..
Alexa Zuber is a dietitian nationally registered under the Commision of Dietetic Registration, and is licensed to practice to her full scope in the state of Ohio under license number LD7601. She may practice nutrition and fitness coaching to a healthy population outside of the state of Ohio. All advice and recommendations given within and outside of the state of Ohio should still be at the acknowledgement of your general health care provider. No given advice is meant to diagnose or cure any health condition.
All recommendations carried out are at the risk of you, the client. Alexa Zuber, and all affiliated companies in reference to The Rooted RD are not responsible for any injuries or health problems you may experience by action or negligence of any program or service in relation to this company.
All payments through therootedrd.com are processed through Stripe in accordance with gethealthie.com and their own privacy policy and terms and conditions. These are acknowledged during checkout. A processing fee is built into each package price reflected on the website. Payments may be made in person, but must be collected prior to a scheduled session.
All virtual 30 or 60 minute sessions must be rescheduled 24 hours in advance of your confirmed appointment time. All cancellations requested inside of 24 hours will result in the loss of your paid session. In home appointments may be rescheduled 5 days in advance without penalty. Cancellations within 5 days up to the day before will result in a $30.00 rescheduling fee. Day of cancellations will result in a $75.00 rescheduling fee or a forfeiture of your full payment. No refunds will be given on any purchases made through the website or given in person for any service.
Cancellations of an automatic billing agreement must be sent via email to Zuber.alexa@gmail.com within 14 days of the scheduled payment day. All cancellations sent in less than 14 days of scheduled payment will result in the billing of one final payment, with subsequent payments being discontinued. Any cancellations given outside of an e-mail to the above address will result in the continuance of your scheduled payment until an appropriate cancellation request is made.
Alexa Zuber and all affiliated businesses believe in protecting your privacy. Any personal information collected via contact form or checkout page is for the sole use of your service with us and will not be sold or given to any third parties. As mentioned in our Terms + Conditions, all payments processed online are processed via Stripe and Healthie. Please refer to their privacy policy as well.
All personal health information is kept in a HIPPA compliant database run by Healthie Inc. This information may only be shared with other healthcare practitioners at the best interest of the client. This information may not be shared with anyone else without written consent given by the client. All communication via the Healthie website falls under HIPPA guidelines.
By submitted a contact form or purchasing a service, you agree to electronic communication from us including but not limited to e-mail, telephone calls, and text messages. These forms of communication can be discussed upon purchase of service.
It is possible to utilize our website without giving any personal information, however information will always be collected when making any purchases. It is at the clients discretion whether to save any payment information to make future purchases more convenient. Credit card information will never be used to process payments through The Rooted RDwithout prior approval. All online payments will receive an email receipt. In person payments may receive a receipt upon request.
